背景軟骨組織工程已成為關節軟骨缺陷功能重建的最佳策略之一。細胞層片技術是一種深具潛力的方法,可用來模仿軟骨組織內分區細胞的分佈,並製造出無外來物的框架支架的分層組織,以改善自體軟骨細胞植入手術(ACI)的結果。震波為一種聲學脈衝,目前被廣泛運用在肌肉骨骼疾病的非侵入性治療。震波治療是一種可能改善軟骨健康的治療工具,但其作用的機制仍然未知。在本研究中,我們旨在利用細胞層片技術和震波治療來改善軟骨組織工程的結果。研究方法關節軟骨細胞是從豬的膝關節中獲得。為了評估分區分層軟骨細胞層片的結果,我們製作了分層軟骨細胞層片,方法為將從淺層區、中層區、深層區分離出來的軟骨細胞,按照組織相應的順序堆疊起來。

而異質軟骨細胞層片則是通過混合分區軟骨細胞所獲得。除了細胞實驗外,我們使用迷你豬膝關節全層軟骨缺陷模型,來模擬自體軟骨移植手術。為了研究衝震波治療對軟骨細胞的功能和訊號傳導,我們使用立體培養豬關節軟骨細胞模型,並使其受到震波治療。評估了不同能量的震波治療對於關節軟骨細胞外基質合成的影響。並且測量了活性氧化物與其他訊號傳遞分子的表現。研究結果相較於異質軟骨細胞層片,分層軟骨細胞層片有較高的軟骨細胞分化基因表現和細胞外基質合成,較低的細胞外基質破壞酶和促炎性細胞因子的濃度。自體軟骨移植手術使用分層軟骨細胞層片,具有較好的軟骨巨觀和組織學結果,所再生的軟骨顯示出更接近於原生軟骨的帶狀結構。另一方面,

震波治療則顯著增加了軟骨細胞的細胞外基質合成,而且不會影響細胞的活力或增殖。其作用機轉是軟骨細胞接受震波治療後,會生成短暫的活性氧化物訊號,並啟動了 ERK1/2 和 p38 的磷酸化以及 Nrf2 的核轉位所達成。結論應用分層軟骨細胞層片於自體軟骨移植手術中,可增加透明軟骨的形成,提供治療關節軟骨缺損手術更好的策略。我們還發現震波治療能有效改善軟骨細胞功能,能通過生成短暫的 ROS 訊號來啟動 Nrf2 的活性,隨後增強軟骨細胞中軟骨細胞外基質合成。震波治療後續也可應用在軟骨細胞的體外培養,利用其非侵入性的物理刺激提高細胞的表現,或應用在臨床當作治療骨關節炎的方法。

在目前椎間盤修補與再生的研究中,絕大部分都著重於髓核的治療,使用注射方式補充各種細胞或生長因子以期達到恢復椎間盤內壓的目的,但此種採用經由纖維環注射的方法就算使用細小孔徑的細針仍會造成纖維環損傷,進而導致椎間盤退化。因此我們認為在椎間盤退化的治療上,修補纖維環的缺損是目前常被忽略,但卻是椎間盤修補與再生能得到長期效果的重要步驟。 目前發展中的纖維環修補技術常以縫線或缺損填塞的方式進行,但這些技術都尚未取得成功的臨床成效。發展組織膠則是另一種修補方法,其優點除了可提供力學穩定度之外,若在其中混入細胞與生長因子將可提高組織再生的效果。本項研究使用明膠與伽瑪-聚穀氨酸並使用碳醯二亞胺當交聯劑製

成水凝膠,並分析其力學強度、細胞毒性與分解速度以評估將其當作注射式組織膠用於椎間盤纖維環的可行性。 第一個實驗先在牛尾椎間盤纖維環上製造針刺缺損後,接著使用水凝膠填塞缺損,然後進行椎間盤內注水測試分析椎間盤內壓力的狀況,發現將濃度 10%的明膠與濃度 3%的伽瑪-聚穀氨酸溶液與濃度 2%的碳醯二亞胺以 10:1 的比例混合後所配置出來的水凝膠可提供足夠的填塞效果,即填塞缺損後可維持與正常椎間盤相當的椎間盤內壓力。第二個實驗再將牛尾椎間盤纖維環細胞種植於水凝膠表面進行細胞培養以評估細胞相容性,當碳醯二亞胺的濃度越高則細胞增殖速度越低,同時製造細胞外基質與去氧核糖核酸的總量越少,但是卻並不影
